Outreach Services
Outreach support is a flexible and personalised way of providing help to adults who live outside of supported living services. Whether in a family home, their own tenancy, or as part of a transition plan, this service enables individuals to receive structured support that fits around daily life.
Outreach input is goal-focused and shaped entirely around the unique context, whether that means maintaining an independent routine, re-engaging with the community, or navigating life changes with confidence. Support can be short-term, long-term, or step-down from more intensive arrangements, depending on what’s needed
Help with managing appointments, tenancy responsibilities, and correspondence
Daily living skills such as shopping, budgeting, cooking, and cleaning
Emotional check-ins and wellbeing support
Confidence-building in community settings
Support with travel and public transport use
Access to education, volunteering, or employment opportunities
Outreach Support
We believe in giving people choice and control. Outreach Support allows adults to receive personalised help in their own homes, fitting support around daily routines and goals. Supported Living offers the chance to live in a dedicated home with built-in support, ensuring stability, safety, and opportunities to grow in confidence.
Outreach Support

Supported Living
Delivered within dedicated supported living homes with housing rights and tenancy agreements
Support can vary from a few hours of support a day, 24 hour one-to-one support, or more if required.
Often used as a longer-term base to develop independence and stability
Includes a transition into accommodation specifically suited for supported living.
Ideal for building a consistent support structure with a regular team.
Both services can work independently or as part of a wider progression pathway, depending on the situation and long-term goals. Our team will work with external professionals, supported individuals and local authorities to figure out what the right pathway is.
Supporting Progress and Independence
Outreach services focus on creating the conditions for long-term progress, not just in practical tasks, but in confidence, emotional wellbeing, and overall quality of life. Plans are reviewed regularly to ensure support remains meaningful and aligned with changing goals.

Since Prudent Supported Living living took on our complex young individual, it has been great. Professionals have felt reassured due to the high standard of support provided to our complex client, as well as the continuous communication, reporting, feedback, and importantly the therapeutic element of the care that’s being provided.
