JOB PURPOSE:
  • To provide support to adults with various needs, including complex mental health conditions, neurodevelopmental conditions and behaviours that challenge.
  • To ensure that the services are well run and promote positive outcomes for people we support.
  • To ensure the services are fully compliant regarding health and safety management and statutory obligations.
  • To monitor and analyse behaviours of concern and other complex needs with the goal of maximising the potential and individuality in accordance with the everyday life principles.
  • To ensure that the staff are well supported through regular team meetings, supervisions and appraisals, and completing the rota.
  • To complete care plans and review them monthly, evidencing person-centred care and service user involvement.

SKILLS/ABILITIES
Required:

  • Excellent knowledge of PBS, safeguarding and mental capacity.
  • Excellent Knowledge of CQC standards and regulations.
  • Strong supervisory skills with the ability to support and develop a team.
  • Proven ability to work on own initiative and as part of a team.
  • Knowledge and understanding of issues relating to mental health, autism, learning disabilities and many other complex conditions which may include behaviours of concern and acts of self-harm.
  • A commitment to a company which provides support to vulnerable individuals, and which is respectful to individuals wishes.
  • Good communication skills both written and verbal. Ability to communicate effectively with individuals we support, colleagues, managers, families and all external professionals.
  • Commitment to training and development.
  • Ability to review and monitor care plans.
  • Proven ability to make decisions and solve problems.
  • Always consider the Mental Capacity Act in the Individual’s best interests
  • Infection control is an employee responsibility, all staff must follow policies and procedures, all staff must be adequately trained in infection control

EXPERIENCE
Required:

  • Experience of working in a multidisciplinary setting.
  • Previous senior management role of at least 4 years.
  • Experience of managing/supervising staff.
  • Minimum of 12 months in a similar setting, ideally with people with Mental health, L/D, Autism, complex behaviours of concern.

EXPERIENCE, SPECIALIST KNOWLEDGE & QUALIFICATIONS
Required:

  • Working knowledge of complex mental health and how to support individuals.
  • Level 5 Diploma in Leadership for Health and Social Care.
  • Registered manager.

Main responsibilities:

Care and support:

  • Implement the philosophies and policies of Prudent Supported Living.
  • Ensure that everyone we support have their needs met through effective assessment and person-centred care planning, review and monitor care plans.
  • Facilitate and support Individual’s decision making, promote choice and involvement.
  • Liaise with other agencies/professionals as necessary.
  • Deal with complaints and concerns, file information and follow up any actions.
  • Update care plans as and when changes arise (e.g medication changes, lessons learned, incidents etc.).
  • Review care plans monthly and complete the care review form. Ensure that the is service user is involved and add information key worker meeting, records of discussions, goals, incident debriefs with IWS, etc..
  • Regularly visit the services to check in with IWS and their team. Complete spot checks. Complete the rotas. Ensure rota amendments are completed and complete hours tracker for previous week. Send the hours tracker and service user hours monthly.
  • Make MAR sheets for homes, monthly and when required. Check medication profiles are up to date all PRN protocols are in place.
  • Review MAR sheets from the previous month, identify any concerns and raise these with the staff.
  • Review monthly keyworker discussions. Identify any care changes, requests, complaints and concerns. Create actions following reviews and assign/complete tasks, print, file, and upload to one drive.
  • Complete monthly audits within the homes checking medication, care plan, and the environment. Complete and assign follow up actions and sign off.
  • Review team leader reports. Highlight changes in support and complete/assign actions. Give feedback to team leaders on the quality of support to facilitate their development.
  • Complete/delegate actions from the Quality Assurance audits and good governance meetings.

Communication:

  • Attend meetings (e.g. MDT, professionals, and safeguarding) and liaise with other professionals – complete and delegate actions to the team.
  • Book in and complete team meetings – share meeting minutes with the staff, file, upload to OneDrive. Complete and delegate actions
  • Answer phone and emails from professionals/homes and complete follow up actions.
  • Arrange support staff and Individual’s we support meetings as and when required.
  • Complete a weekly report and send to the director, registered manager, and quality and compliance manager.

Marketing:

  • Actively market the Company and promote positive personal/professional profile within the local community, always ensuring a good reputation of the Support services.
  • Support with the transitions of new service users. Attend transition meetings, complete needs assessments, and assist the registered manager with completing care plans and setting up the homes. 

Training and Development:

  • Support team leaders with their skills and development and complete their supervisions (e.g. supervising their supervisions, reviewing their meetings with staff).
  • Assign team leaders weekly tasks and ensure that they are completing what they are responsible for.
  • Maintain professional knowledge and competence.
  • Attend mandatory training and complete eLearning modules as and when required.
  • Ensure you read your employee handbook thoroughly and ensure you read a copy of the codes of practice as required.

Health and Safety:

  • Understand and ensure the implementation of the support services health and safety policy and emergency fire procedure.
  • Report to the office manager or maintenance any faulty appliances, damaged furniture, equipment or any potential hazard.
  • Promote safe working practice within the supported setting.
  • Complete monthly audits of the service environments.
  • Ensure that the accident folders are completed by operational support and that all RIDDORs completed.

General:

  • Ensure all information of a confidential nature gained in the course of duty is not divulged to third parties.
  • Notify the line office manager and director as soon as possible of your inability to report for duty, and on your return to work from all periods of absence.
  • Adhere to all company policies and procedures within the defined timescales.
  • Ensure all equipment is clean and well maintained.
  • Carry out any other tasks that maybe reasonably assigned to you.
  • Carry out on-call duties on a rota basis.
