How Our Referral Process Works

  1. Complete Our Referral Form
    Share a few details about the individual’s background, needs, and current situation so we can understand how best to support them.
  2. Consultation & Assessment
    Our team will be in touch to explore the referral in more depth. We’ll carry out a friendly, needs-led assessment to determine the right level of support.
  3. Personalised Support Plan
    If Prudent is the right fit, we’ll develop a tailored support plan that focuses on the individual’s goals, independence, and well-being — and support a smooth transition into their new home.
